| Reason for Aircraft Loss | Starboard outer engine failed shortly after takeoff, which was feathered. The crew decided to turn back and used their H2S equipment to establish their position but it failed to function. While setting up his Gee rig instead, the aircraft let down through the clouds and immediately flew into high ground near Pateley Bridge, Yorkshire |
